SUPPORT SERVICES
A child in your classroom may need additional support for a variety of reasons. If you, as a classroom teacher, feel that you have observed, assessed and made appropriate modifications for a child, but a further plan seems necessary, you may request a Child Study conference to help you determine the next step. At this meeting appropriate staff members will discuss the childs educational history and current learning strengths and weaknesses and strategies about other instructional approaches or supports to meet the students needs.
Chapter 766 - Special Education Referrals
A student may be referred to Special Education by a social worker, principal, other school staff members, or by his or her parents. When a referral is made, an Evaluation Team is established; the team consists of a team chairperson, school psychologist, special education teacher, speech and language pathologist, classroom teacher(s), parents, and in some cases the principal. All participants will hear the results of a standardized battery of tests and discuss their own observations and impressions of the students strengths and weaknesses. If she/he is found to have a disability and be eligible for special education services, then these team members participate in formulating an individualized educational plan (IEP).

504 Accommodation Plan
A student who has been referred for a special education evaluation may be found to have a disability but does not meet the eligibility requirements to receive special education services. If the team determines that the student needs an accommodation to assist him/her to receive the full benefit of the educational program, a 504 accommodation plan will be written. This plan outlines the accommodations the student needs and is signed by the parents. A 504 accommodation becomes part of the students permanent record.
